Locality Hubs
Locality Hubs provide a patient-facing service that offers integrated, community-based care from joint teams on a locality footprint and at a neighbourhood level.
Key functions are:
- Locality Care Coordination Centre: to understand which individuals are accessing urgent and emergency care services and to coordinate the intermediate care pathways to keep citizens safe in their own home, support discharge from hospital settings and move to proactive case finding to keep people well in their own homes.
- Integrated Same-Day Urgent Care: to offer flexible, same-day urgent care pathways to individuals to better manage end-to-end care of their condition and prevent the need for admission or multiple professional contacts.
- Proactive care and Long term condition management: to proactively identify and support individuals who could benefit from access to specialist advise and clinics and to anticipate care needs far earlier e.g. via remote monitoring or care plan optimisation.
- System surge capacity: to be able to provide additional capacity within the locality to deal with surges in demand such as primary care provision when needed, vaccinations / immunisation demand.
